How to distinguish by clinical features?

In oropharyngeal dysphagia, there is difficulty initiating swallow and drooling, aspiration, and cough

for oropharyngeal dysphagia or dysphagia due to upper esophageal pathology, UGI endoscopy is not helpful because when u insert the endoscope from the upper esophagus, it can miss it. That's why, we prefer a nasopharyngeal laryngoscope (NPL) or barium esophagogram.
