Acute epididymitis

causative bacteria: gonorrhea and chlamydia if the age is less than 35 while coliform bacteria in a person aged more than 35.

clinical features

epididymitis may be associated with urethritis causing urethral discharge and pain at the tip of the penis

relief of pain with scrotal elevation

investigation: urine R/E M/E, urine c/s; NAAT for gonorrhea/ chlamydia if age is less than 35.

Treatment:

if gonorrhea is suspected: ceftriaxone+ doxycycline

if gonorrhea is less likely, then fluoroquinolones (levofloxacin)

supportive measures: scrotal elevation, NSAIDs
