Indication:

if any 1 of the following: 1. coagulopathy ( PT INR >1.5, APTT more than 2 times ULN, platelet <50,000

                                         2. spinal cord injury, head trauma, major burns

                                         3. mechanical ventilation for more than 48 hours

                                         4. GI bleeding or ulceration in the last 12 months

  if any 2 of the following: 

                                       1. ICU stay for more than 7 days

                                        2. glucocorticoid therapy

                                       3. sepsis

                                       4. occult gi bleeding for more than 6 days
